Secondary adrenal insufficiency is caused by a deficiency in the secretion of adrenocorticotropic hormone, which stimulates the secretion of cortisol in the brain. It can occur after a tumor in the pituitary gland or hypothalamus or surgery for a pituitary adenoma, and can also occur after long-term steroid administration.
Representative symptoms of adrenal insufficiency include general weakness, decreased appetite, fatigue, and gastrointestinal symptoms (nausea, vomiting, constipation, diarrhea). Weight loss, hypoglycemia, hypotension, alopecia, muscle or joint pain, etc. may also occur, and primary adrenal insufficiency is characterized by hyperpigmentation of the skin. If the disease progresses rapidly (adrenal crisis), abdominal pain, fever, cramps, severe dehydration, and electrolyte abnormalities may occur, and in severe cases, loss of consciousness, shock, and even death may occur.
Blood tests are used to measure electrolyte, cortisol, and adrenocorticotropic hormone concentrations, and additional CT or MRI tests may be performed depending on the results.
To treat adrenal insufficiency, supplement the hormones that are lacking in the body with steroids (such as hydrocortisone or prednisolone). In stressful situations such as surgery, trauma, fever, and infection, the dosage of the medication being taken may need to be increased, so consultation with the attending physician is necessary.
In acute adrenal insufficiency such as adrenal crisis, hydrocortisone is administered intravenously, and dehydration, electrolyte abnormalities, and low blood pressure are corrected with fluid and glucose injections.
Since cortisol normally increases in response to stress, patients with adrenal insufficiency may experience more severe hormone deficiency symptoms in stressful situations such as surgery, trauma, fever, and infection. Therefore, in such situations, you should increase the dose of steroids by 2-3 times from the previous dose, and if you cannot take the medication under severe stress, you should visit the emergency room and receive steroid injections.
