① If you have accessed the Government 24 website, please search for [Certificate of medical record]
② Click the [Apply] button.
③ Please enter your basic personal information.
④ Please check the items in the application details.
Receipt method – Online issuance / Receiving institution – Neighborhood community center / Number of copies – 1 case
⑤ Click the [Apply for civil complaint] button.
⑥ Click [Service Application Details] > [Online Application] > [Print Document] button and print it with a printer to complete the issuance.

Select [Save as PDF] to save.
