– Persons with a body mass index of 25.0 kg/㎡ or more as a result of national health examination, blood pressure of 120 (systolic)/80 (diastolic) mmHg or more, or fasting blood sugar of 100 mg/dL or more (health management type) Hypertension registered in the primary care chronic disease management pilot project , diabetic
– A person who has established a care plan by registering for a primary care chronic disease management pilot project
